Please help.

My answer to the both of the above is
Good effective communication amongst everybody involved in the individual's health and social care.

Apart from this simple answer, I have gone blank.  I cant think of anymore, but I supoose more would be required as these are two separate questions.

You need to be seen or show/explain where/how you have done this.

implementation of the plan - You discuss with the individual and family members how and when the care plan will be put into effect and how it will be carried out.

changes to needs and preferences - This might be written or orally on a regular basis or at a review meeting.
