Hi stheronell,

1 Example - the hoist is broken.

2 What would you do?

3 Would you do what she wants or ether seek immediate advice/support or advise her that you must follow her risk assessment this time but that you will report her choices for a new risk assessment.

4 When the required equipment is not available, when the individual's condition/ability has changed/deteriorated, when you have not been trained to deal with a specific situation eg new equipment etc.
