does anyone know what action to take if the individual's wishes conflict with their plan of care?..am totally stuck on this one Sad

Hi nightnurse,

Remember the individual has the right to make choices and to change their mind. However, you have a duty of care (and the right to be kept safe) so you must not change the care plan without carrying out a risk assessment. Report the individuals wishes so that a risk assessment can be undertaken and a new care plan agreed.
