Hi nicoleM09,

Work in partnership with the individual and others to identify the individual’s specific communication needs.

This is something that you either need to be seen doing or you could write a reflective account. It means working with others ie from other agencies such as a nurse, speech therapist, day centre workers etc, and with family or others involved in caring for the individual to form a partnership of carers who cooperate for the benefit of the individual.

Just write an account of when you have done this. For example you arranged for a speech therapist to observe the individual in various locations including your own setting and the day centre. Then you arranged a meeting for everyone to discuss issues, problems and solutions. This would require cooperation from all involved.

Hope that helps

Aunty Sue
