have been giving some work and my brain has went dead coming back to it from being off hope some one can help me

How do you support individuals and key people to fill in any forms of paper work to enable them to access the agreed services and facilities?


How do you support individuals to identify any changes necessary to improve the outcomes from the use of services and facilities?


How do you encourage individuals and key people to give feedback on which services and facilities have been beneficial and those that have been less helpful to their well- being?

Hi scottielass,

What do you do? Do you first establish what help they need eg understanding it, reading it, filling it in? Do you then go through the form? Do you then go though each question and provide the required support? When completed do you check their answers with them? And so on.

The next two go together. First you provide time and a suitable environment (private, relaxed, no interruptions etc) to talk then you discuss the services and facilities provided (going through each one) and ask for feedback on them from the individuals and key people ie are they working? could there be improvements? what would make them better? etc. You encourage them to express their own views and repeat them back to them to ensure clarity. That sort of thing.
