just started my Diploma 2 and haven't got a clue - finding it very hard (well I am old) I need help with Unit 4222-232 Outcome 5, question 6. Monitor the individual throughout the activity so that the procedure can be stopped if there is any adverse reaction - can someone please give me some pointers.

Hi Mell!  You're not old - you're experienced and there's no substitute for that in this job!  Grin The learning outcome you are describing is one which should really have been seen by an assessor or witness within the workplace but it is not a common thing to happen which may be why you've been asked to write about it - so, your written account will need to reflect upon a real experience and will need be about a time when you were assisting someone to be moved and handled and something went wrong as you were watching (monitoring) the move being carried out so that you had to stop what you were doing - maybe rearrange a hoist sling or the battery went flat or the person became agitated and distressed.  Don't forget to explain who you told about the issue and where it was documented.
