Can anyone help with this questions. Have been struggling for a while now and it is driving me crazy.


Evaluate one method or 'tool' used in your organisation to plan for end of life care. You can do this by finding out how effectively it delivers the following aspects. For each one, note any improvements that could be made in delivering the care. (2.4)

Assessment method or tool: 

a) Discussions as end of life care approaches 

b) Assessment, care and planning review   

c) Co-ordination of care 

d) Delivery of high quality services in different settings 

e) Care in the last hours of life 

f) Care after death 

I have been searching the internet and I am just confused even further and no matter what i type in the search engine I get tons of results
