hello ive just started my nvq3 ime finding it hard to under stand how i am supposed to write about some of the topics
hsc 358 identify the risk of skin break down and under take the appropriate risk assessment can any one help please

Hi Diane,

Read the PCs, think about examples of what you actually do (not what you would do) and write a reflective account (or reflective accounts) to fit them. For example:

To cover HSC 358a PCs 1 and 2

Mrs X is 83 and recovering from a stroke and is bedridden. Mr Y is 80 and is in his wheelchair most of the time. Mrs X and Mr Y have been assessed as being vulnerable to skin breakdown.

As a result of her stroke Mrs X has very little movement, this can result in pressure on vulnerable parts of her body which may cause her to develop pressure sores. Mr Y is overweight and incontinent and this has led to him developing skin rashes around his abdomen and buttocks.

and so on.
