Hi can someone help with this question please

two ways of making sure that the history, preferences, wishes and needs of an
individual using the service are recognised in support or care plan

i know all this information is written up in the individuals care plan but it is asking for two ways

Hi pinkcandy,

How do you make sure that what is put in the care plan is correct and what the individual wants? How do you find out if the support that is given is what the individual wants?
